I agree to let my recorded interview be used and grant the right to broadcast or publish any resulting interviews, my likeness and my name to Blue Ribbon Stories, the Northwest Washington Fair, Falling Star Studio, Sarah Sakurazawa and their agents on the air, online, in print and other media for editorial or promotional purposes.
I agree to grant Blue Ribbon Stories, the Northwest Washington Fair, Falling Star Studio, Sarah Sakurazawa and their authorized representatives permission to record me on photography, film, audio recording and/or video. I further agree that any or all of the material photographed or video recorded may be used, in any form, as part of any future publications, broadcasts, online content, brochures, and print or online promotional materials including social media outlets.
I hereby give my permission for Blue Ribbon Stories, the Northwest Washington Fair, Falling Star Studio, Sarah Sakurazawa and their authorized representatives to use photos and/or videos of me for social media, marketing and promotional purposes.
Use of said print and multimedia materials shall be without payment to me of fees, royalties, special credit or other compensation.
I hereby release and discharge Blue Ribbon Stories, the Northwest Washington Fair, Falling Star Studio, Sarah Sakurazawa and their authorized representatives from any and all claims and demands arising out of, or in connection with, the use of the photographs and video footage, including and without limitation, any and all claims for libel or invasion of privacy.
